Unable to Submit LOA

Issue

I am unable to submit Leave of absence form (LOA)

Error Messages Present: Error when submitting Form. Please Contact Help Desk. The Server cannot find the form mapping for path: /form/HR/LOA/EmployeeID. Contact your administrator. (ARK95L: GYD1BF)

Environment

• Leave of absence form (LOA)

Information

This error occurs when the form times out while filling out the leave of absence.

Resolution

1. Delete the EMPLID field
2. Retype your ID
3. Click Submit
