Unable to Submit LOA

Issue

I am unable to submit the Leave of Absence (LOA) form. When attempting to submit, the following error message appears:

Error when submitting Form. Please Contact Help Desk. The Server cannot find the form mapping for path: /form/HR/LOA/EmployeeID. Contact your administrator. (ARK95L: GYD1BF)

Information

This error typically occurs when the form session times out while you are completing the document. This causes the link between your Employee ID (EMPLID) and the submission path to "break" in the browser's active memory.

Resolution

To refresh the form mapping without losing all your entered data, follow these steps:

  1. Navigate to the Employee ID (EMPLID) field at the top of the form.
  2. Delete the current ID number from the box.
  3. Manually re-type your Employee ID into the field. This triggers the form to re-verify the mapping path.
  4. Scroll to the bottom and click Submit again.
Note: If the error persists after re-typing your ID, you may need to clear your browser cache or open the form in a fresh Incognito/Private window.
