Survey Request

Information

This form is to be completed by those who wish to send a survey to GRCC Faculty, Staff, or Students. The decision as to whether to allow the survey will be made by the Deans Council using the following guidelines. The rationale for this process is to protect students from excessive survey requests.

Key Features

  • The survey results will be used to improve practice at GRCC and/or support students to be more successful in achieving their goals.
  • The survey results will not be used or reported as partial fulfillment of degree requirements for any non-GRCC class or program, conference, or journal article. (Requests for permission to collect data for these uses must go to the Institutional Review Board).
  • These guidelines do not include surveys sent to subsets or segments of GRCC students. Please contact the Dean of Institutional Research and Planning for additional details.
  • A detailed plan for how the data will be reported and used must be provided.
  • Student surveys will be limited to 2 surveys per semester with at least 30 days between survey requests. (Not including student evaluations of faculty as prescribed through the Faculty Contract).
  • Surveys that collect data which inform policies and practices related to those goals within the college Strategic Plan will take precedence.
  • Established college-wide surveys related to GRCC Ends and Indicators will take precedence including student satisfaction surveys, student engagement surveys, and campus climate surveys.
  • This process is intended for GRCC faculty and staff only. Requests from those external to GRCC must be reviewed through the Institutional Review board. Requests from GRCC students will not be considered.
  • Only aggregated data will be provided to the researcher (no individual student data). Individual student data may only be collected in order to award incentives.
  • All surveys must be sent from Survey Monkey or another similar product with a GRCC license (i.e. no “free” accounts.
  • Survey incentives, if appropriate, must follow established guidelines set by the Provost and the Purchasing Policy.
  • These guidelines cover all surveys who are intended to be completed by all GRCC students no matter what format is used (i.e. paper/pencil, electronic, mailed, posted to social media).
  • The content of all surveys must be approved by the Director of Institutional Research.
