Completing Identity Verification

Information

Identity Verification is a common requirement necessary for students to complete their FAFSA.

Environment

Process

This verification cannot be submitted electronically. You must complete it either in person at GRCC or by using a notary. You can find the document within your Student Forms Account (opens in a new window).

Option 1: In Person (Recommended)

To complete the form at the main campus:

  1. Visit the RJF Student Services front desk on the first floor of Raleigh J. Finkelstein (RJF) Hall.
  2. Ask to speak with a Financial Aid representative.
  3. Important: You must sign the form in the presence of the representative. Bring a valid government-issued photo ID (such as a Driver's License or State ID).
    1. Note that a military ID is prohibited from being photocopied by law, so cannot be provided for this purpose. Temporary ID's such as a temporary drivers license does not contain a photo and also cannot be accepted.
  4. If you cannot print the form beforehand, a representative can print it for you when you arrive.

Option 2: By Mail (Requires Notary)

If you cannot visit the campus, you must have the form notarized:

  1. Log into your Student Forms Account (opens in a new window).
  2. Click on the Needs Action box and select the Identity Verification requirement.
  3. Click the Download button to save and print the form.
  4. Take the printed form to a Notary Public. You must sign the form in their presence.
  5. Mail or otherwise physically deliver the original notarized document along with a copy of the valid photo ID presented to the notary to:
    GRCC Financial Aid
    143 Bostwick Ave. NE
    Grand Rapids, MI 49503

Tip: Many local institutions, such as the Grand Rapids Public Library or your personal bank, often provide notary services for free or a small fee.
