Completing the Identity and Statement of Educational Purpose Form

Information

The Identity and Statement of Educational Purpose form is a mandatory document used to verify a student's identity. It confirms your understanding that federal student aid must be used solely for educational expenses. This form is often requested as part of the financial aid verification process and must be finalized before your aid can be processed.

Process

This form cannot be submitted electronically. You must complete it either in person at GRCC or by using a notary. You can find the document within your Student Forms Account (opens in a new window).

Option 1: In Person (Recommended)

To complete the form at the main campus:

  1. Visit the RJF Student Services front desk on the first floor of Raleigh J. Finkelstein (RJF) Hall.
  2. Ask to speak with a Financial Aid representative.
  3. Important: You must sign the form in the presence of the representative. Bring a valid government-issued photo ID (such as a Driver's License or State ID).
  4. If you cannot print the form beforehand, a representative can print it for you when you arrive.

Option 2: By Mail (Requires Notary)

If you cannot visit the campus, you must have the form notarized:

  1. Log into your Student Forms Account (opens in a new window).
  2. Click on the Needs Action box and select the Statement of Identity requirement.
  3. Click the Download button to save and print the form.
  4. Take the printed form to a Notary Public. You must sign the form in their presence.
  5. Mail the original notarized document along with a copy of your valid photo ID to:
