Identity Verification and Statement of Educational Purpose

Information

The Identity and Statement of Educational Purpose form is a document that verifies a student's identity and ensures that they understand that federal student aid can only be used for educational purposes. The form is requested as part of certain verification requirements. The form must be completed before financial aid can be processed.

Process

Students can complete the form in person in front of a Financial Aid Representative or by use of a notary. If the document is notarized, the Financial Aid Office must receive the original document with the notary stamp/seal via mail or dropped off in person. The form can be found in your Student Forms Account.

In person:

  • Stop by the RJF Student Services front desk located on the first floor of the RJF Hall.
  • Ask to speak with the Financial Aid representative.
  • If you don't have the form printed, have someone help you do that from one of the computers in their office. The form can also be printed by a Financial Aid Representative when you appear in person to complete it.

Mailed:

  • Please log into your Student Forms Account.
  • There will be a box labeled, "Needs Action." Click on the box and then the "Statement of Identity" requirement.  A "download" button will appear. Click on the download button and save the form to your computer in order to print it.
  • Print the form, bring it to a notary to be completed and then mail the original with a copy of a valid driver's license or State ID to:
